Dedication to ensuring individual health and safety, willingness to respectfully collaborate with the interdisciplinary team (nursing, clinical, management, residential directors, health care providers, guardians).
Duties included but not limited to: Physical assistance with ambulation, physical assistance with all activities of daily living (showering, toileting, incontinence care, dressing, meal preparation in accordance with individual dietary guidelines, CPAP set up), proper use and documentation of assistive devices (motion detectors, bed rails, video/audio monitor, gait belt, helmet, shower chairs), training and implementation of individual specific protocols (seizure protocols, vagus nerve stimulator use, head banging protocols, CPAP protocol, dietary guidelines, ADL guidelines, fall protocol, constipation protocol), accompanying individuals to medical appointments with the ability to provide pertinent information and advocacy .

Functional Responsibilities:
- Supervises and transports clients during assigned hours, this may include, job search, medical appointments, weekly recreation, shopping trips, and other necessary trips.
- Supports and teaches clients new skills including, but not limited to, those specific to goals stated in the Individual Service Plan.
- Provides a positive, supportive role model for the client.
- Administer medications.
- Documentation and timely submission of all progress notes, weekly summaries, attendance, and all other client documentation deemed necessary by the company.
- Attends all weekly, monthly, etc. meetings specific to the client.
- To support individuals in attaining the highest quality of life while maintaining their safety across all settings.

High School Diploma or GED- Strong verbal and written communication skills.
- Valid driver’s license and reliable transportation that can accommodate individuals safely to their destination if needed.
- Able to drive house wheelchair vans with individuals.
- Ability to act appropriately in the event of an emergency.
- Must be able to pass required certifications in the allotted time set forth by the agency.
- Map Certification must be completed with 6 months of hire
- Must pass a criminal and driver’s check.
Technical Competencies:
Basic computer skills (email, databases, internet, etc…), copy, scan, and fax.
Working Conditions:
- Ability to work shifts to accommodate business needs.
- Must be willing to accompany individuals in the community (Religious services, hospitals, etc...)
