Job description
Manages the centralized care coordination center. Facilities a collaborative approach to patient-centered care, ensuring the needs of patients, their families and caregivers are met. Ensures effective 24/7 call management and prior authorization and referral processes. Manages the nurse triage process to positively impact the delivery of healthcare for the organization.
Education and Training
- Associates degree or equivalent experience required.
- Clinical designation (CMA, LPN, RN) preferred.
Experience
- Two years of management experience.

- Excellent computer literacy
