New Glarus Home is seeking a MDS Coordinator.

Are you passionate about improving the quality of life for seniors? The primary purpose of the MDS Registered Nurse is to maintain the medical records in accordance with State and Federal Regulations as well as professional standards of practice and facility policies and procedures to ensure complete, timely, and accurate medical records.

Essential Job Functions:

• Enters MDS+ data in the computer
• Maintains the confidentiality of medical records by ensuring compliance with chart sign-out forms and authorization for release of information
• Collects and assesses nursing information on new admits, change in condition and quarterly assessments for MDS
• Starts Care Plans on new admits
• Attends Care Plan Meetings weekly
• Attends Medicare Meetings weekly
• Reviews the Dx, MAR, TAR, CNA flow sheet for accuracy
• Assists with nursing cares/duties as needed
• Processes requests for medical information as directed by the DON or administrator
• Maintains clean work area
• Has access to confidential information and maintains confidentiality
• Attend in-services as directed
• Other duties as assigned by the Director of Nursing

Educational/Experience:

• Requires a degree in nursing from an accredited college or diploma from nursing school
• One year experience in medical records (MDS, RAP, RAI, Care Plan development) desired in a hospital, long term care facility or other related health care facility preferred
