Registered Nurse MDS Coordinator

In this critical position as an MDS Coordinator at Riverside Nursing and Rehabilitation Center you’ll ensure that our residents receive the proper care they deserve and need. Your work will have an important and meaningful impact on the quality of care that our resident receive.

We are seeking a qualified candidate who has experience as an MDS Coordinator responsible for completion of the Resident Assessment Instrument in accordance with federal and state regulations and company policy and procedures. Acts as in-house case manager by considering all aspects of the resident's care and coordinating services with physicians, families, third party payers and facility staff.

Responsibilities of MDS Coordinator:

• Accurate and thorough completion of the Minimum Data Set, Care Area Assessments and Care Plans, in accordance with PDPM and current federal and state regulations and guidelines that govern the process
• Proactively communicates with Administrator to identify regulatory risk, clinical trends that impacts resident care, and any additional information that has an effect on the clinical and operational outcomes of the center
• Collaborates with the facility’s staff to select the correct assessment of our residents and acts as an in-house Case Manager
• Demonstrates an understanding of MDS requirements related to varied payers including Medicare, Managed Care and Medicaid
• Ensures timely submission of all Minimum Data Sets to the state data base and ensures that the necessary follow-up action is taken
• Facilitates the Care Management Process by engaging the resident and all other appropriates staff members
• Analyzes QI/QM data with the Director of Nursing to identify trends on a monthly basis and serves on, participates in, and attends various committees of the facility.

Requirements of RNAC MDS - RN Assessment Coordinator:

• Registered Nurse with current, active New Jersey
• Minimum two (2) years of clinical experience in Nursing Home
• Prior experience as an MDS coordinator
• Experience with PDPM
