Provide wholistic care for patients that extends to the spiritual level by praying with patients and families. Chaplains are available to meet with patients, families and team members 24 hours a day, 7 days a week to provide spiritual support.
Help develop a brand new $150 million community hospital and medical office building that will provide services such as Emergency Services, Medical/Surgical, ICU, Women’s Services, General Surgery, Orthopedics, Interventional Cardiology and Gastroenterology.

The role you’ll contribute:
The Care Management Manager is under the general leadership of the Care Management Facility Director and has the responsibility of the departmental daily operations and supervision of team members.
The Care Management Manager is responsible for, but not limited to: Coordination of Care, Centers for Medicare & Medicaid Services (CMS) Conditions of Participation, Social Services implementation of processes based upon data management, accountability for Length of Stay, Excess Days, Avoidable Days, Readmission Prevention, and Discharge Planning. In addition, the Care Management Manager is responsible for department engagement, customer experience, compliance of regulatory standards, and employee evaluations. The Care Management Manager supports professional and collaborative relationships with the Interdisciplinary Team to sustain process improvement and attain hospital metrics.

- Knowledge of medical necessity for appropriate patient status and level of care
- Understanding and ability to implement mitigation techniques to attain hospital metrics
- Ability to analyze, evaluate, and monitor data
- Ability to foster positive Interdisciplinary Team relations and encourage employee engagement
- Strong professional verbal and electronic communication
- Ability to work independently and/or part of a team
- Strong technology skills
- Understanding of Managed Care and Value Based Programs
- Ability to negotiate optimal resolution of issues
The expertise and experiences you’ll need to succeed:
EDUCATION AND EXPERIENCE REQUIRED:
- Registered Nurse: Must have a BSN
- 3 years in direct Care Management/Utilization Management experience in an acute care setting
- Masters in related field
- 2 years of supervisory/lead/manager level of responsibility experience
