Job description

We are seeking a Licensed Vocation Nurse (LVN) interested in providing one-on-one skilled nursing care and treatment to patients in their homes. They must use a comprehensive and patient-specific care plan that prioritizes the patient's health and independence. Candidate must have good communication skills to collaborate with physicians, patients and their families, nurses, therapists, social workers, and other members of the patient care team. Our ideal candidate will be spending quality time with the patient and must be compliant and punctual with shifts and electronic charting.

HIRING FOR THE FOLLOWING CASES:

• RIALTO - MON-FRI 7AM - 3PM

*** $500 sign-on bonus ($250 after 90 days and $250 after 180 days)***

Most of our patients require (2) or more of the following Nursing Skills:

• Ventilator
• Tracheostomy
• Suction
• Gastrostomy
• Seizure Precautions
• Oral, SubQ, IM medication administration

**Rate: Starting at $29/hr**

Credentials Required:

• Valid professional licensed in California
• Personal Identification Card (ID)
• Social Security Card
• Permanent Resident Card, or work permit
• Physical exam
• Tuberculosis Results have to be within a year or negative results upon hire
