PanCare of Florida is seeking an experienced LMHC or LCSW (Licensed Mental Health Counselor or Licensed Clinical Social Worker ) to join our Behavioral Health team in Freeport, FL.

This position is in a Behavioral Health/Family Practice setting, face to face and via Telehealth. This individual would serve as a member of the primary care and case management teams, providing direct client mental health services and coordinating with the case management team to ensure that clients are linked to all appropriate services.

Requirements:

• Supervises family support caseworker and associated clerical case management support staff.
• Reviews case worker's assessment of clients as appropriate and coordinates the development of a care plan.
• Completes a thorough mental health/drug abuse assessment of clients as indicated and refers to outside sources for services as needed.
• Reviews and facilitates, if appropriate, mental health referrals from other sources such as primary care, school health, etc. Offers alternative recommendations where necessary.
• Coordinates with eligibility determination and directs the process of linking clients to a comprehensive array of needed services. Ensures referrals for patients requiring services not offered at the health center.
• Supervises ongoing client assessment and care plan development; monitors individual client outcomes.
• Provides on-site prevention, intervention, and treatment within their scope of practice, to clients presenting with several mental and psychosocial illnesses, including depression, substance abuse, stress, uncontrolled anger, and alcoholism.
• Utilizes techniques and strategies such as warm hand-off, short-term focus therapy, solution focus therapy, and cognitive behavioral therapy.

Qualifications:

• Master's degree in social or behavioral sciences.
• Licensed as a Licensed Mental Health Counselor or Licensed Clinical Social Worker in Florida.
• Experience as a therapist, counselor, or case manager.
• Current Cardiopulmonary Resuscitation Certificate (CPR)
• Bi-Lingual (Spanish Speaking a plus!)
