• 
  Makes skilled nursing visits to patient residence as scheduled by CM RN and manager. Provides all aspects of patient care as directed by the RN in patient’s individualized care plan.
• Evaluates the effectiveness of treatments and medications, provides and reinforces patient/family education.
• Reports on daily visits, findings and interventions on each patient to their Case Manager RN.
• Performs patient care with the general assistance and supervision of the case manager according to company polices.
• Completes treatments and procedures within scope of practice and acceptable limits and in accordance with the plan of care.
• Accurately documents care in the patient's clinical record in a timely manner per McLaren policy.
• Actively seeks professional development.
• Participates in case conferences or IDT meetings as directed.
• Attends staff meetings and all mandatory in- services as established by company policy.
• Adheres to ethical nursing standards. Maintains confidentiality and privacy of patient family records, finances, and health status.
• Promotes customer service and patient satisfaction.
• Participates in rotation of afterhours nursing on call services with RN back up.
• Adheres to infection control policies and procedures.
