Our services include nursing, physical, occupational and speech therapy, social workers and home health aides. Specialized programming is available for those with COPD, pain, and congestive heart failure.

Job Summary

The Licensed Practical Nurse is responsible for utilizing the nursing process to provide for optimum quality and continuity of nursing care to patients in the comfort of their residence. This individual demonstrates the core nursing competencies established by the agency to ensure effective and efficient care on an ongoing basis.

Candidate Profile

• Current, unencumbered AZ LPN/LVN license required.
• A minimum of 2 years LPN/LVN experience required.
• A minimum of 1 year Home Health LPN/LVN experience required.
• Wellsky/Kinnser experience preferred.
• Valid driver's license with proof of insurance in accordance with state requirements. 100% travel required.
• COVID-19 vaccination required, or reasonable accommodation based on disability, medical condition, or religious belief that prevents vaccination.
• Able to educate patients and family on multiple disease processes and have good medication knowledge and wound care experience.
