CHC Solutions Inc is currently seeking an experienced full-time Licensed Practical Nurse (LPN) to join our team in our Pittsburgh (Robinson Township) office.

In the Licensed Practical Nurse (LPN) role, you will do the following:

• Provide primary point of contact for patient including clinical triage, initial calls and follow up.
• Complete initial review of new orders.
• Obtain patient updates regarding product usage and changes in plan of care.
• Provides consultation and education to external customers and patients regarding enteral nutrition, wound care, ostomy, urological and incontinence products.
• Provide support to various teams within the organization including but not limited to evaluating product usage, specific supply needs and documentation.
• Collaborate with AR/Authorization departments to ensure payor specific regulations/guidelines are handled appropriately.
• Act as liaison between referral source, patient and customer service.
• Attend and participate in patient and family-centered support groups and community events.
• Perform other duties as assigned/required.

EDUCATION and/or EXPERIENCE

• Must hold license in active standing in the state where service is provided
• Five years of clinical nursing experience in acute care, home health, or long-term care required
• Two years direct experience caring for patients with wound care, ostomy and/or urological needs

CERTIFICATES, LICENSES, REGISTRATIONS

• RN License OR LPN License
• Drivers License
