LPN Licensed Practical Nurse Coordinator

Job Summary:

You will be an integral part of the Assisted Living care team and provide ongoing support in the monitoring, coordinating, managing and delivery of care to residents through collaboration with the nursing staff, community caregivers, outside providers, residents and resident families. In addition to supporting the Assisted Living care team and function, the LPN Coordinator acts as ‘point-of-contact’ and addresses routine and/or emergent needs of Independent Living residents.

Responsibilities include:

• Collaborate with Assisted Living Manager to provide supervision and oversight for resident care and day-to-day needs of staff
• Support Assisted Living Manager with coordinating care with outside providers and pharmacy, including order clarification/requests and follow-up (fax, phone, telehealth)
• Confirm documentation is completed each shift in Point Click Care (PCC) and internal communications binder, including PHI and care orders
• Complete nursing audits related to medication and care orders through PCC
• Participate and provide information in Quality Assurance (QA) programs and efforts
• Provide emergency and acute nursing needs for Independent Living residents
• Work collaboratively with social services function to help address psycho-social challenges impacting residents’ day-to-day health and quality of life
• Conduct medication and safety assessments, including annual medication assessments and OTC medication reviews

• Assist in the tracking and reviewing of incident reports

Requirements:

• LPN license in good standing in WA state
• Current CPR card
