Job Description
As a Kaiser Intake Specialist you will manage the referral inquiry and admission process and provide customers with facility-related information via phone conversation and follow-up. You will coordinate and facilitate on-site assessments and referral to admission conversions. You will have an assigned location to position the company as the leader in services and provider of choice in the market. You will work with hospital discharge planners, internal sales team, managed care organizations and referral sources. You will review and analyze trends to identify opportunities in patient types while maintaining updates on bed availability and facility services at all times. You will continuously communicate with your building, starting with decisions of admission recommendations, room and bed selection, and the special needs of new residents to the facility staff, to ensure a smooth transition. It is important for our Kaiser Intake Specialist to maintain a relationship with qualified referral sources and contacts. You will be responsible to treat everyone with dignity, respect, compassion and integrity. Required (Must have):
  • Nursing degree from an accredited college or graduation from an approved LVN or RN program
  • 1-3 years experience in direct patient/resident care
  • Knowledge of clinical standards of practice
  • Ability to maintain professional communication and accountability to represent the company
  • Capability to maintain regular attendance and ability to adapt to change
  • Compassion toward residents and their families
Preferred:
  • Previous admissions, assessment, and telephonic nursing experience
  • Experience in Skilled Nursing Facility (SNF) environment
  • Ability to learn regulations, eligibility, and reimbursement governing long term care (Medicare, Medi-Cal, and Medicaid)
  • Strong financial and analytical skills
  • Computer skills
  • Ability to work flexible hours, as admission responsibilities may dictate
