Summary

Under the direction of the Program Manager, the Intake Coordinator performs duties for clients requesting services, providing exceptional customer service in managing client placement or coordinating any means necessary referrals to an external provider.

Responsibilities include but are not limited to the following:

  • Meet with youth and families in the juvenile justice system that are referred by the Court or Juvenile Probation Officers
  • Receive and process referrals from the courts
  • Exhibits a thorough understanding of the program and wrap-around services, policies and procedures
  • Works closely with Family Team facilitator on assessments and referrals
  • Conduct assessments and screenings in accordance with protocols established
  • Collaborate with Probation and the Court on individual case plans for youth
  • Coordinate and participate in wrap-around staffing's for new youth referred to the Redeploy Program
  • Promotes a proactive atmosphere of “prevention” rather than “reaction". Educate justice involved youth on physical and mental health
  • Works to increase level of independence of youth justice involved
  • Must be present for court services, and provide proper testimony when necessary
  • Provides an atmosphere of respect and compassion for youth and demonstrates good rapport
  • Proficient in Youth Assessment Screening Instrument (YASI)
  • Demonstrates professional judgment and decision-making skills related to youth, staff and program matters
  • Demonstrates knowledge of legal issues and civil rights relevant to work setting

HIPPA: Full access to client information. Confidentiality must be maintained at all times.
Quality Improvement: Participate in ongoing quality improvements efforts of the agency.
Diversity/Cultural Awareness: Ability to work effectively with diverse communities and constituencies.

Qualification Requirements: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education & Experience:

  • Bachelor's degree in a related field from an accredited university required
  • Experience working with youth, justice system. and families preferred
  • Youth Assessment Screening Instrument (YASI) experience preferred, but willing to train
  • Bilingual in Spanish is a PLUS, but not required

Valid Illinois driver's license is required
