This position is responsible for the timely scheduling, coordination and recording of new client assessments and the development of client care plans; reporting, and providing on-going existing client assessments for Individuals with Intellectual or Developmental Disabilities. The I/DD Care Coordinator will work directly with Direct Support Professionals (DSP) to ensure that action plans are completed and Individual Support Plans (ISP) are followed. The purpose of the program is focused on person-centered support that will enhance the lives of those who utilize and work in the ODDS system. The program will provide quality support so people with intellectual and developmental disabilities have a good life in the communities where they live, work and play. A critical component of this role is to demonstrate the agency’s culture of compassion, accountability, and results by displaying the highest standards of integrity, superb follow-through, consistency, first-rate service, and exceptional client satisfaction.

Responsibilities:
Work with Care Advisors to obtain all applicable plans of support from the Services Coordinator/Personal Agent prior to starting services.
Perform initial assessments for prospective clients with ODDS Services Coordinator or Personal Agent and ISP team members to determine level of care needed for ADL/iADLs, identified safety and behavior risks, and ISP goal completion.
Identify, document and maintain records of Individual’s outside supports i.e. PCP, family members.
Complete new Care Plans and applicable health/safety protocols based on the assessment and supports identified in the ISP (in coordination with New Horizons RN for nursing services clients).
Create Action Plans for ISP goals/desired outcomes to ensure that goals are set and being worked towards, following the clients Individual Support Plan (ISP).
Provide person-centered training on all applicable support documents to assigned DSPs prior to working with the client, and annually thereafter.
Supervise DSPs assigned to clients on caseload and provide training, coaching, and corrective action as needed in collaboration with I/DD Program Director and HR.
Salary: $50,000 - 55,000 DOE
Qualifications:
Minimum of 2 years experience, working with Individuals who experience intellectual or developmental disabilities. Supervision experience preferred.
Self-starter who knows how to identify opportunities with recommended solutions
Creative problem-solving skills
Possess strong written, verbal, and presentation skills with diverse populations
Ability to multi-task and stay calm and work as a team player in a fast paced environment while ensuring attention to detail
Strong computer skills and typing skills
Valid Driver’s license. OIS, CPR/FA certificates preferred.
