Job description
- Assist in scheduling and re-scheduling patient appointments to primary care and specialists when requested by care team.
- Book exam rooms at Commonwealth Community Care (CCC) when requested by care team.
- Procuring orders and referral requests to agencies and vendors for DME/Medical Supplies and Home Health Aid as requested by care team.
- Follow up on orders and referrals by submitting documentation when needed to support the request for services.
- Triage and transcribe patient calls from Member Services and Clinical Review Unit (CRU) and route documented message to appropriate care team member in EPIC.
- Print, scan and fax referrals or documents in need of care team signature for service requests.
- Create shadow charts in eCW system.
- Mail welcome packets to new enrolled patients.
- Update Excel spreadsheet with new patient information.
- Weekly communication with Health Information Management (HIM) to provide new enrolled patient demographics.
- Support team in tracking Key Performance Indicators (KPI) on Excel spreadsheet and send communication around metrics.
- Support management with performing eligible patient chart reviews and patient utilization reviews.
- Research and provide care team with benefit information regarding Masshealth covered services.
- Attends weekly team meetings and supports by booking rooms.
- Fax prescription orders to pharmacies at request of clinician.
- Submit requests for transportation (PT1) via Masshealth Customer Web Portal.
- Schedule Masshealth transportation rides with MART and Cab Rides as needed.
- Submit Medminder referrals when requested by clinicians.
- Attends/facilitates biweekly eligibility meeting to discuss and provide updates on ACO unaligned patients on behalf of team members.
- Other related duties maybe assigned
- Associates degree required, Bachelor's Degree preferred
- 3 -5 years of administrative experience in a medical or healthcare related setting
- Experience working in a multi-cultural setting
- Excellent written and verbal communication skills
- Working knowledge of Microsoft Office applications (Excel proficiency basic-intermediate)
- Excellent organizational skills
- Knowledge of some medical terminology preferred
- Ability to prioritize workload and to multi-task effectively
- Ability to problem solve with minimal supervision
- Enthusiasm and commitment to teamwork
- English required, bilingual preferred
