We are now seeking a part-time or full-time Nurse Practitioner within our Hospice division.

Hospice Nurse Practitioner Essential Functions

• Complete initial/comprehensive assessment of the patient/family to determine hospice needs
• Complete ongoing assessments of the patient’s physical status and the psycho-social status of the patient/family
• Initiate and develop a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family and the attending physician
• Make necessary revisions to the IDG plan of care as the patient’s/family’s status and needs change
• Actively participates in IDG meetings
• Documents an individual patient's progression in end-stage disease in the patient/family record as appropriate
• Provides assistance to patients and families in goal setting and care plan development with end-of-life discussions
• Plans care utilizing community resources and act as a liaison by relaying information to other health care professionals and agencies to provide for continuity of care
• Completes other assignments as requested and assigned

Hospice Nurse Practitioner Requirements

• Graduate of an accredited school of nursing with an advanced degree in nursing - either MSN or DNP
• Licensed to practice as a Nurse Practitioner in the state of Michigan. The license is active and in good standing
• Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years
• One (1) year of hospice/palliative care experience preferred
• Active CPR certification
• Reliable transportation with a valid state driver's license and current car insurance
