Job description
Providence Care is currently looking for a Hospice Care Navigator / Account Executive to build relationships, provides information regarding end of life services to attending physicians, facility staff, hospitals, patients and families in the Rock Hill, SC and surrounding areas.
Responsibilities:
- Develops and maintains appropriate sales and referral strategy
- Successfully attains weekly referral and admission goals per sales strategy.
- Creates and maintains a physician/referral contact list and build relationships with key personnel.
- Fosters communication between hospice and facility staff; and, coordinates services and provision of end of life resources on behalf of the patient and family.
- Plans, organizes, and schedules programs for civic and social community groups.
- Represents the agency at Health Fairs and other events in the community.
- Makes routine facility, doctor, and hospital visits to directly market Providence’s services.
- Communicates with the medical staff, nursing personnel, and other department supervisors.
- Works beyond normal working hours and on weekends and holidays when necessary for special projects.
- Is involved in community/civic health matters/projects as appropriate.
- Attends and participates in continuing educational programs.
Qualifications
- Bachelor’s degree is preferred but not required. A study emphasis in social work, psychology, counseling, marketing, or related field is preferred.
- A proven success record in outside sales
- A minimum of two years of sales experience dealing with data, people, program development, and/or marketing is required.
- Experience in a hospice, home health, healthcare, or similar setting is required.
- Must possess the ability to deal tactfully with personnel, patients, family members, visitors, physicians, government agencies/personnel, and the general public.
- Ensure that reports required by regulatory agencies are prepared and submitted within established timeframes.
- Must be knowledgeable of hospice practices and procedures, as well as laws, regulations, and guidelines that pertain to hospice care.
- Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
- Must possess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.

Pay: From $60,000.00 per year
