Homeless Case Manager/CSP Provider

SUMMARY: Case Managers maintain responsibility for a caseload and provide case management services for clients who have been diagnosed with a serious mental illness and are homeless or at risk for homelessness. This position will provide housing case management (Housing Support Services and Housing Sustaining Services) for 7 units of permanent supportive housing at White Pines Apartments, 5 units above the Outreach Center, and provide/administer Bridges vouchers. This position interacts with community providers such as Carlton County Health and Human Services, Cloquet HRA, United Way, Lakes and Pines CAC, FDLTCC, and others. The case manager receives training in the HMIS (Homeless Management Information System) and the VI-SPDAT (Vulnerability Index – Service Prioritization Decision Assistance Tool) and complete all other required documentation including a Housing Service Plan. Will conduct street outreach, meeting homeless individuals in community settings. The case manager develops excellent working relationships with community partners, housing management, and landlords. In addition to housing, the case manager might also maintain part of a caseload providing Adult Rehabilitative Mental Health Services (ARMHS) as needed. The case manager works in a team setting and participates in a multidisciplinary team meeting and receives clinical supervision.

MINIMUM QUALIFICATIONS:

- B.A. degree in human services plus 2000 hours of supervised experience (1-year) in adult and/or children’s mental health setting OR

- Completed Master’s Degree in Human Services or Behavioral Science. OR

- Meet Case Manager Associate requirements.
