We are currently looking for a Homecare Liaison (LPN or RN) to join our team. This position will be responsible for developing relationship with facility, coordination of referrals to homecare & imputing in shell and intake note. This will be co-signed by RN, BDM Responsibilities:

We are currently looking for a Homecare Liaison (LPN or RN) to join our team. This position will be responsible for developing relationship with facility, coordination of referrals to homecare & imputing in shell and intake note. This will be co-signed by RN, BDM
- Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
- Coordinates referrals to home care to promote continuity of care
- Provides individualized counseling to patient and family regarding available home care services
- Establishes and maintains professional working relationship with hospital staff, physicians, and VNAHG staff
- Ensures compliance with regulatory and statutory requirements
- Provides back up coverage to assure availability of intake coordinator services
- Adheres to the organization’s policy in regards to absenteeism and appearance
- LPN or RN
- Valid FL driver's license
- Homecare experience
- Homecare sales experience preferred
- Excellent interpersonal and organizational skills
- Strong client focus, assertive, decision making skills, articulate, and creative in problem solving
- Strong decision making skills
- CPR Required
Working Conditions/Physical Demand:
- Working indoors and may be required to travel outside to business community locations as needed
- Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and driver’s license.
