- DMJ Home Health Is a rapidly growing home health agency that is currently hiring new energetic staff! Our services are designed to provide quality home care that is tailored to the unique needs of our patients and families under a comprehensive plan of care established in conjunction with our health care team and their primary physician.
**Full-time per Diem, PRN, Part-time per Diem**
JOB SUMMARY:
- Provide direct patient care to assess their medical condition, functional capabilities, limitations and restrictions.
- Establish and administer a treatment program with specific and individualized goals.
- Knowledge of how to instruct other support staff on patient’s limitations and coordinate care as appropriate.
- Responsible for accurately document all patient encounters, examinations and discharges.
- Must have the ability to work effectively in both independent and team environments.
- Capable of accurately interpreting patient charts medical history including diagnostic and laboratory reports.
- Have knowledge and competency in examination and assessment procedures with regards to strength, ROM, accessory joint mobility, sensory perception, function, balance, and appropriate special tests.
QUALIFICATIONS:
- Licensed as a Occupational Therapist in Illinois and graduated from a Occupational therapy approved curriculum by the American Physical Therapy Association or The Committee on Allied Health Education and Accreditation of the American medical Association
- A minimum of one (1) year clinical experience, home health experience preferred
- Computer skills adequate for data entry of clinical documentation
- Current CPR certification
- Current Illinois Driver’s License and proof of current auto insurance
- Must be able to read, speak and understand the English language
