Bachelor’s Degree in Social Work, Psychology, Public Health, Sociology, or other related field; MSW preferred. Knowledge of HIV infection related social and emotional issues, and demonstrates competency in working with culturally diverse, low income, or no income clients and special populations. (required). Experience in assisting clients in social services and healthcare access, as well as knowledge and experience in providing HIV/AIDS education and information is preferred, including working knowledge of medical terminology particularly as it relates to HIV disease preferred. Bilingual (Spanish) strongly preferred.
This position requires providing service to the following age population(s) 18-80 in a manner that demonstrates an understanding of the functional/developmental age of the individual served.
- Provide full range of Medical Case Management (MCM) services to people living with HIV (PLWH) assigned to caseload, including Standard MCM and Comprehensive MCM services with goal of improving healthcare outcomes for PLWH
- Function as part of the IDC team to ensure the provision of high quality services
- Assess clients’ needs; develop and implement individualized plan of care
- Link clients to needed services, such as medical, mental health, substance abuse, and support services
- Maintain contact with CMCM and SMCM clients at the minimum specified time intervals and minimum monthly service units
- Utilize a strengths-based, empowerment-focused, and trauma-informed clinical approach to client interactions
- Conduct home visits and meetings in the community as needed
- Collaborate with other service providers, advocate for clients, exhaust all available resources
- Provide information, education, treatment adherence, and referrals as needed
- Serve as a resource for clients and the community for HIV information
- Complete written documentation of client interactions and services provided within two business days
- Attend trainings, professional development, and in-services
- Participate in regular supervision meetings
- Maintain awareness and knowledge of current benefits, entitlements, and resources available to clients
- Other duties as assigned
