Contract: 13-weeks Full-time, 40 Hours Guaranteed

Setting:

• Field staff all work remotely after training.
• Most of our patients will be in Chicago area with many patients around the Oaklawn, Lake Zurich, and Lombard areas. Our patients can be located up to the Il/Wi border and down to the Il/Indiana border and as far west as Aurora.

Requirements:

• Active RN in IL
• Active BLS
• At least 1 years of Home Health Experience
• Excellent Infusion Skills
• Technically Savvy, Strong Computer and Phone Skills
• Must be able to lift up to 20lbs
• Must be willing to travel
• Strong EPIC experience required
• Reliable Transportation
• Active Driver's License
• Strong Communication Skills

Additional Job Responsibilities related to Home Hospital:

• Responsible for Caseloads of Patients in specialty areas.
• Conducting initial assessments to obtain detailed physical, and environmental data.
• Establishing plan of care in regulation with physician orders
• Advises and consults with family
• Provides interventions and on going assessments of patient care
• Completes documentation according to regulations
• Must be able to provide care to CHF patients and identify critical care elements within each patients specific care plan

Knowledge and Skills Required:

• Proficiency in clinical skills and the ability to work under direction and make appropriate judgement
• Work well with physicians and other staff members
• Strong interpersonal skills
• Must have tolerance for differences and of multicultural populations
• Must be able read assess patient data and provide care based on the specific health data retrieved.
• Must be able to carry up to 50lbs
• Must be able to perform a sliding transfer of 150lbs with a second person present
• Must be able to make strong judgment calls regarding patient safety
