Job Description

• Knowledge of Personal Care Services (Medicaid) and state requirements – Contracting and Regulatory
• Comply with clients rights and responsibilities
• Ensures client and employee privacy and confidentiality pursuant to HIPAA Privacy Rules
• Completed initial evaluation of assistance needed by client to achieve activities of daily living
• Monitoring hospitalizations and/or all suspensions
• Assessing and confirming service plan task meet clients’ needs
• Ensuring task are not duplicated with any other services received by the client
• Ensure Coordination of Care with required forms and communication
• Scheduling according to service plans, authorized hours and required days per week
• Determining which meals (breakfast, lunch, dinner) w/meal prep and frequency
• Recruits and Hires Personal Care Attendants – Complies with clients choice if choice meets employee hiring requirements
• Supervises PCA ensuring client's individualized service plan meets requirements, and client’s needs are met
• Provides training with Electronic Visit Verification confirming attendant fully understands and comprehends processes with monitoring in and out for scheduled hours.

Requirements

• Must have dependable vehicle, valid driver's license, and auto insurance in accordance with the laws of the State.
• Customer Satisfaction – Having passion, treating all (Attendants, Clients, Staff and Peers) with respect and dignity
• Ability to cope with ill, disabled, and dying patients
• Delivering continuum of care to clients in need  Ability to sit, stand, bend, lift 50+ pounds, and move intermittently
• Ability to have physical safety and bloodborne pathogen protocol to minimize illness and/or injury
• 2 years of college or two years of supervisory experience in a health related setting.
