Job description
NFI Massachusetts, Inc., a non-profit human service organization, is currently seeking a Family Support Worker based out of Northshore Massachusetts, for our community-based services working with youth and their families.
The program provides family-centered services in the home or community setting and can include one or more of the following: family therapy, therapeutic support services, parent support, comprehensive assessment, counseling, case coordination, and crisis intervention services for youths and families. Services and service intensity are highly individualized to meet the unique and varying needs of each youth/family and have the capacity to be fluid and change during the course of treatment.
Each service component can operate as “stand alone,” as part of a larger NFI team, or can be effectively integrated into larger outside treatment teams that might include hospitals, state agencies, outpatient services, residential treatment programs and other community based programs.
Responsibilities Include:
- Conduct and/or participate in intakes.
- With guidance from youth, family, caregivers and other supports creates a strength based client focused assessment and treatment planning.
- Educates and supports caregivers in navigating systems of care and how to access appropriate care for their child/grandchild/etc.
- Assist and support caregivers in problem solving and advocacy skills.
- Assist in locating and accessing resources.
- Work collaboratively with treatment team to support goal development, crisis planning and problem solving.
- Support caregiver in development strategies for self-care, identifying respite care providers and accessing respite care.
- Provide in the moment modeling and coaching to increase parenting skills and sense of competency.
- Acts as liaison between program, family, outside service providers, the court, and referring agency as determined by the treatment plan.
- Participate in weekly program and staff meetings.
- Writes necessary progress notes, aftercare plans, service plans, and reviews as defined by the program contract.
- Assist other staff in crisis prevention and crisis management.
- Perform all duties in accordance with the agency’s policies and procedures.
- Completes other related duties as assigned.
Requirements
- Must have lived experience as a parent/caregiver of a youth with a significant social, emotional, behavioral health, medical or trauma related needs and experienced in navigating child and family serving systems.
- Bachelor's preferred
- Excellent interpersonal, organizational and writing skills required.
- Flexible schedule including evening and weekend availability.
- Valid driver’s license as travel is required.
Hourly Rate: $18.50/hr
Benefits for full time employees include: Medical Insurance, Dental Insurance, Vision Insurance, 403(b) Retirement Plan, 3 weeks paid vacation time, Paid Sick Leave and Tuition reimbursement up to $2500 per fiscal year
