La Familia Medical Center is seeking a Family Nurse Practitioner to serve as a PCP to conduct primary medical diagnostic care and service, refer consultations for specialty care and provide education and preventive care advice to our patients in the Hialeah and Little Havana.
Responsibilities include but are not limited to:
Provide diagnosis, treatment, therapy and management of patients according to current standards of care.
Responsible for accurate, timely, and complete documentation in patient records in accordance with set policies.
Conduct or direct education effective communications to patients, families, and other health care providers.
Assess and interpret patients’ data relevant to successful treatment.
Provide ongoing monitoring of patient; identify deviations from expected progress; assess change in patient status and collaborates with interdisciplinary health care teams as deemed appropriate.
Job Types: Full-time, Part-time
Pay: $76,313.87 - $128,591.33 per year
