Aviva Health is currently recruiting for a Family Nurse Practitioner in our Roseburg Clinic.

Functions as Family Nurse Practitioner in an out-patient primary health care clinic, serving individuals with barriers to health care of Douglas County. Under the direction of the Chief Medical Officer, the Family Nurse Practitioner assumes the responsibility of routine patient care including: assessment, diagnosis, and evaluation of injury and/or illness, and ordering of routine exams. Utilizes skills in providing diagnosis and treatment of acute and chronic health problems, along with providing planning and problem solving with clinic staff and volunteers. Provides care consistent with policies and procedures of the organization.
QUALIFICATIONS:
Graduate of an accredited school of nursing with advanced preparation as a Family Nurse Practitioner; Master’s degree preferred.
Current licensure in the state of Oregon as a registered nurse, with certification in the state of Oregon as a Family Nurse Practitioner with prescriptive privileges.
· Holds current DEA license.
· One-year professional nursing experience in a clinic setting.
· Preference given to bilingual applicants.
AGE SPECIFIC CRITERIA:
Has knowledge and skills to provide services within scope of practice which may include the following age specific groups:
§ Neonates (birth to 1 year)
§ Children (1 year to 12 years)
§ Adolescents (13 years to 17 years)
§ Adults (18 years to 64 years)
§ Geriatric (65 years and above)
Job Type: Full-time
Pay: $108,401.00 - $115,052.00 per year
