JOB DESCRIPTION

Primary Duties and Responsibilities:

• Providing quality care, direction, guidance, and help to individuals with daily activities such as bathing and bathroom functions, feeding, planning menus, dressing, grooming, taking medication and/or administering treatments, and housework.
• Help individuals make and keep appointments, provide or arrange transportation to run errands and serve as a companion
• Help individuals achieve agreed-upon goals to improve quality of life and integration into the community
• Provide written daily updates of each shift’s activities using an online or paper documentation platform
• Observe the individual’s condition and monitor the surroundings to ensure their safety and success. This includes monitoring of equipment and materials
• Perform other duties as assigned
