Discharge Planners serve as a liaison between the inpatient setting and the assigned clinician at the Outpatient site. The Discharge Planner will work with Level I Inpatient Providers to review presenting clinical information, identify resources for successful discharge to the community, and monitor TXIX adult GMH/SA Behavioral Health Recipients (BHRs) follow up with the outpatient provider. The Discharge Planner works under the supervision of the Health Center Director or Site Manager.

Discharge Planner Duties include:

• Activities will be performed in both inpatient and outpatient settings.
• Connect individuals receiving Level I inpatient services with ongoing TERROS Outpatient treatment upon discharge.
• Work with BHRs while in the hospital, will work with transitioning BHRs, the hospital unit staff, the Outpatient team, family, RBHA care management, PCPs and Health Plans as
• Clinically indicated to ensure effective transition to Outpatient or community services, and
• Reduce the risk of readmission to hospital, reduce the utilization of Crisis services and improve the coordination with the PCP and TERROS psychiatric provider.

• Required: Minimum of a High School Diploma or GED
• Preferred:
  • A bachelor’s degree in a behavioral health field.
  • A bachelor’s degree in any field, plus one year of work experience in a behavioral health service delivery:
• Must possess a combination of behavioral health education and work experience totaling a minimum of four (4) years
• Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy
• Must have a valid Level 1 Arizona Fingerprint Clearance card or apply for one within 7 working days of assuming role.
• Must pass a TB Test.
• Proof of Covid-19 Vaccination required
