Job Summary:

North Kern Tulare Hospital District is looking for an experienced Social Services Director to join our dedicated team of nurses offering Skilled Nursing Care to our residents at Delano District Skilled Nursing Hospital.

Duties/ Responsibilities:

· Plan, develop, organize, implement, evaluate, and direct the social service programs of this facility.

· Demonstrates authority, accountability, initiative, and responsibility of the department.

· Assist in the development, administering, and coordinating of department policies and procedures.

· Identify medically related social and emotional needs of the resident.

· Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures according to new/updated regulations.

· Provides in-service/training in revisions of policies as needed.

· Review department policies and procedures independently and at QAPI/Quality Assurance meetings monthly. Apply necessary recommendations and/or changes.

· Develop and implement policies and procedures for the identification of medically related social and emotional needs of the resident.

· Manage and oversee ancillary services for all residents.

· Ensure compliance and record keeping.

· Manage complaints, concerns, and resident appointments as needed.

· Assist, approve, and/or authorize family requests for letters and other miscellaneous requests that may arise.

· Accept and complete other assignments as directed by the Administrator.

· Assist in standardizing the methods in which work will be accomplished. Develop or revise forms as needed for efficiency and appropriate documentation.

· Work with the facility’s consultants as necessary and implement recommended changes as required

· Develop and maintain a community and social services referral file of agencies and organizations that aid residents. Refer residents and family members as needed to such agencies. Document referrals in social service notes.

· Prioritize tasks and job duties according to safety, regulations, compliance, needs, and deadlines.

· Performs other related duties as assigned.

Required Skills/Abilities:

• Must be fully vaccinated against COVID-19, including having received booster shots and have proof of vaccination status.
• Experience in Skilled Nursing/long term care Environment
• Experience with PointClickCare
• Attention to detail and problem-solving skills
• A creative mind with an ability to suggest improvements

· Excellent verbal and written communication skills.

· Excellent interpersonal and customer service skills.

· Excellent organizational skills and attention to detail.

· Excellent time management skills with a proven ability to meet deadlines.

· Proficient with Microsoft Office Suite or related software.

· Preferred bilingual.

Education and Experience:

· At least two years of supervisory capacity in a hospital, nursing care facility, other related medical facility, or facility with counseling services, therapy services, conflict resolution, and/or social work.

· Bachelor’s Degree in social work, human services, or another related field.

Physical Requirements:

· Prolonged periods sitting at a desk and working on a computer.

· Prolonged periods of standing and moving

· Must be able to lift to 50 pounds at times.

Salary:

· Salary: $30.00-$40.00 per hour

Work Schedule

· Monday through Friday, 8a-5p with occasional Overtime
