Position Summary

Accurate Home Care has an opening for a Director of Home Health Services to lead our home health program. At Accurate Home Care, our Home Health team aids in empowering our patients to stay at home and recover. This strong team of home health professionals manages the care of our patients while building relationships and ensuring a positive patient experience.

Pay & Perks:

• Monday – Friday; salaried position working 40+hours/week; on-call availability to support your team by phone
• $105,000-$115,000 per year

What will I be doing?

• Plans, coordinates, delivers, and oversees the delivery of outcomes of required healthcare related services/training programs on a routine basis; monitors both skilled and non-skilled healthcare services.
• Quality Assurance and Performance Improvement analysis, including quality validation of visit documentation completed by Nurse Case Managers.
• Directly supervises Nurse Case Managers in the implementation, coordination, maintenance, evaluation, and termination of the individualized plan of care, analysis outcomes, medical records, and physician orders
• Provide regular direct patient care through admission assessments, care planning, goal setting, evaluation, coordination of care, and emergency/safety plans.

What skills do I need to bring?

• Valid license as a Registered Nurse in the state of Minnesota.
• Recent Oasis experience (within the last 12 months)
• Recent Direct Patient Care (within the last 12 months)
• 2 years Home Health experience
• 1 year supervisory experience
• Working knowledge of Microsoft Office suite.
• Must show proof of COVID-19 vaccination or medical or religious exemption
• Ability to read, write and speak fluently in English.
• Education and experience in the scope of services offered by the agency.
• Basic Life Support (BLS) CPR course for healthcare providers through American Heart Association or Red Cross renewed at recommended frequencies.
• Maintain confidentiality in accordance with applicable laws and rules.
• Valid driver’s license and proof of adequate insurance.
