Job description
  • Review all paper intake forms and enter the information received into STARS
  • Enters meal participation, education/recreation, health promotion, etc. activity units into STARS
  • Enters all case assistance and information and referral units into the STARS system from case assistance staff’s respective Case Assistance/ Information & Referral Excel Spreadsheets
  • Enters and maintains all client information in STARS
  • Runs and reviews weekly STARS reports to ensure all areas of clients’ STARS computer files are correct and up to date and notifies the Program Director of any trends in utilization or lack thereof
  • Maintains up to date clients’ and community resources’ emergency contact information spreadsheets and distributed them to center staff
  • Provides, if needed, case assistance and information and referral services
  • Contacts clients on STARS Active Non Served Clients List to ensure their well-being and encourage their participation in center’s activities
  • Conducts telephone reassurance and friendly visiting
  • Tracks NSI status for all members to ensure they are updated annually
  • Maintains and updates the health/wellness library
  • Enters six-month cycle menus into Simple Servings in the absence of the Program Director
  • Submits monthly STARS Client and Unit Report and Meal Breakdown Report to the Program Director and Director of Programs
  • Provides general clerical duties to include, but not limited to: typing, photocopying, faxing, mailing and filing
  • Distributed in person/emails health and wellness information to members’ monthly
  • Perform all tasks requested by the Program Director to support the team functioning of the Older Adult Center.

Pay: $44,235.00 per year

  • Microsoft Excel: 1 year (Preferred)
