Job Title: Advanced Practice Provider
General Summary of Duties: Responsible for patient care, clinical oversight of qualified healthcare professionals like nurses and medical assistants. Responsible for patient care in the outpatient office setting, skilled nursing, assisted living and long-term care settings. Provide traditional primary care, integrative medicine, chronic care management, and palliative care to patients with a focus on integrative medicine and a philosophy that views a patient as a whole person, to improve outcomes.

Supervisory Responsibilities: NA
Overtime Status: X Exempt ☐ Non-exempt
Major Responsibilities/Tasks:
Assessing patients with acute, chronic, and non-emergency problems.
Completing a full medical assessment including but not limited to Past Medical, surgical, Social and Family history.
Diagnosing and treating injuries, illness, and chronic conditions
Ordering, performing, and interpreting lab and diagnostic testing
Counselling patients on diet, hygiene, preventative healthcare, and lifestyle management
Creating and managing treatment plans
Prescribing Medications and therapies
Providing basic Integrative Health care to the patients
Ensuring that patients attend routine and follow up visits
Encourage patients to get the recommended health screenings and assessments
Make referrals to specialists, if necessary
Manage the overall care of the patient and collaborating with the entire healthcare team
Comply with all medical laws and regulatory standards
Document all treatment and assessments in the medical record
Submit appropriate billing according to documentation supplied
Manage patients’ treatment in outpatient office, skilled nursing, assisted living and long-term care facilities

6. Ability to communicate difficult conversations professionally
Ability to empathize with patients, families, and caregivers
Requirements:
· Active, unencumbered Pennsylvania Nurse Practitioner or Physician assistant license
· CPR Certification
Work Environment:
· Moderate physical activity. Requires handling of average-weight objects up to 25 pounds or standing and/or walking for more than four (4) hours per day.
· Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment.
· Will work with blood or blood-borne pathogens and will require OSHA training
