GENERAL STATEMENT OF FUNCTIONS

Responsible for documentation, supervision, scheduling and instruction of participants in a community setting.

SPECIFIC DUTIES AND RESPONSIBILITIES

- Coordinates transportation and other support for participant as necessary

- Implements program curricula

- Supports participants in meeting their objectives in their Person Centered Service Plan

- Maintains documentation on services provided and participant progress

- Delivers individual and small group learning experiences

- Identifies strengths, desires, talents, skills and challenges of their assigned participants. Provides this information to Case Managers and Program Coordinator

- Makes recommendations for participant goals and objectives

- Reviews pertinent intake and progress information

- Supports Program Coordinator in planning daily/weekly activities based on

Individual PCSP.

- Under direction of Coordinator prepares bi-weekly participant schedules

- Applies behavior modification programs

- Advocates for the participant

- Attends and participates in assigned meetings and in-service trainings

- Helps to develop community sites for work and activity experience

- Other related duties as assigned

QUALIFICATIONS, EDUCATION AND EXPERIENCE

- 21 years or older

- Valid California Driver's License; acceptable driving record

- State minimum required auto insurance

- Reliable transportation to transport participants to CIS activities

- Proficient knowledge on the use of Google Drive, emails and tablets

- CPR and First Aid Certification within 30 days of employment

- Ability to communicate well with others

- Ability to role model socially appropriate behavior and attitude for participants

- Availability and ability to work a flexible schedule

- B.A. in Social Services or related field preferred; or A.A. in Social Services or related filed preferred with at least two years’ experience in Social Services; or a High School diploma with at least three years’ experience in Social Services required.

- Ability to express ideals clearly both written and orally

- Have good English grammar skills

- Must pass a criminal record clearance

- Must be vaccinated against COVID-19

PHYSICAL ABILITIES

- Ability to physically demonstrate or lead a small class

- Sitting, walking and standing are a part of daily routine while instructing or observing the participant

- Ability to tolerate extremes in temperatures

- Ability to lift 50 pounds

- Fingering skills to complete paperwork

- Sense of feeling for temperature and safety

- Coordination for lifting, carrying, climbing and balancing

- Coordination for stooping, reaching, crouching, and crawling

- Good vision and hearing to relate effectively with participants and community

MENTAL ABILITIES

- Capacity to interpret instructions and communicate to participants

- Math skills to complete required paperwork

- Ability to meet deadlines and ensure timely submission of information requested

- Good judgment to advocate, recognize safety hazards, and apply appropriate behavior modification techniques
