Essential Duties & Job Responsibilities:

  • Provide service coordination, focused on connecting vulnerable individuals to a variety of service pathways, including hospitals, mental health treatment facilities, emergency shelters, motels and crisis services, etc.
  • Track clients' progress through various service pathways and monitor client outcomes.
  • Utilize evidenced-based best practices, including motivational interviewing, trauma informed care, and harm reduction when engaging individuals in their recovery.
  • Utilize valid triage and service prioritization tools and establish reliable pathways while providing expedited access to needed services.
  • Ensure the confidentiality of every client served by the New Day program.
  • Act as a liaison to health, social and civic services (e.g., hospitals, substance use programs)
  • Meet individually with clients to assist with transition to services based on clients' needs.
  • Transition individuals into permanent supportive services or proper placement based on the clients' needs.
  • Coordinate necessary and related data collection, reporting, and report progress to New Day program director.
  • Maintain accurate and up-to-date records of client charts.
  • Write and submit monthly reports on program outcomes.
  • Attend staff meetings with the New Day team to staff cases and strategize interventions.
  • Provide ongoing support to New Day clients who have transitioned to required services.
  • Maintain contact with service providers to encourage referrals and ensure effective coordination of ongoing services.
  • Participate in weekly supervision to review cases and issues related to level of service.
  • Present and foster a trusting, professional, and nonjudgmental relationship with all clients served by New Day.
  • Follow Veterans Multi-Service Center & New Day policies and procedures in working with the population served

Education and Experience:

A Bachelor’s degree in Human Services, Public Administration, Social Work, or related field; or equivalent background with five (5) years of experience working with homelessness, poverty, housing, mental health and human services.

  • Former Street Outreach experience is a plus.
  • Strong relationship builder and communicator with a customer service focus;
  • Ability to learn, understand and adhere to Program guidelines, policies and procedures;
  • Ability to work cooperatively and collaboratively with all levels of employees, management, and external agencies to maximize performance, creativity, problem solving, and results;
  • Ability to learn best practice models, including mental health recovery, harm reduction, trauma informed care, housing first;
  • Ability to communicate effectively both verbally and in writing;
  • Knowledge of and experience in working with HMIS and other spreadsheets and databases;
  • Proficient in MS Office (Word, PowerPoint and Excel) and computer usage to include email, internet and presentations;
  • Experience leading small and large meetings or workgroups a plus;
  • Familiarity with recovery, mental health, homelessness, poverty and human services;
  • Possess valid driver's license.
