Overview:
Reports to Covid Community Corp Coordinator for providing outreach, information and education on vaccines, their availability to all targeted communities in Monmouth County. The CHW will work with clients to address social, economic, and health challenges in order to support COVID prevention, testing, and vaccination to better align community members, residents and stakeholders with a healthier place to live, work and play.

Responsibilities:
  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
  • Conducts outreach and education activities for community members including residents and stakeholders, especially those disproportionately affected by COVID.
  • Establishes a network of trust within the community’s residents in order to offer needed resources and access.
  • Acquires and compiles lead and health data within the community and its residents to assist COC and CFHI Leadership reaching health outcomes
  • Adheres to the organization’s policy in regard to absenteeism and appearance

Qualifications:
  • Associate’s/Bachelor’s degree preferred but not required
  • Relevant experience gained through professional or personal domains; experience facilitating and conducting community education events is helpful
  • Bilingual; Spanish preferred
  • Knowledge of cultural and lifestyle diversity issues with ability to network with a specific population
  • Excellent verbal and written communication skills
  • Strong organizational and interpersonal skills
  • Valid and current driver’s license
