If so, we have an exciting opportunity for you to work as a full time Community Health Worker in the Services for Youth & Families Department!

What You Will Be Doing
Working as a Community Health Worker in the Services for Youth & Families Department, you will be responsible for providing outreach in the community to at risk populations as well as providing direct assistance with identifying social detriments of health resources and the navigation of those resources. You will assist individuals utilize resources and complete applications for programs they’re eligible for. You will schedule appointments and provide excellent customer service which may include phone calls, in person meetings or visits in a community setting. You will use your knowledge of community resources to connect individuals with services and programs such as housing, food, education and healthcare to ensure their basic needs are met.

What We Require
A high school diploma or GED with 1 year of work experience in mental health, healthcare, or community resources is required. You must be able to prioritize tasks and manage time in an effective and efficient manner. You must be able to work independently, maintain professional boundaries and be detail orientated. Basic computer skills including word processing, spreadsheets, database management and e-mail is required. Travel throughout the Kalamazoo area is required. Valid Michigan Driver's License is required. Lived experiences with behavioral health are desired.

Physical Efforts – Job demands include prolonged sitting and standing as appropriate. May occasionally require light lifting up to 25 pounds, stooping, kneeling, crouching, or bending as appropriate. Requires coordination of hands and/or eye/hand/foot.

Working Conditions – Office environment with noise from computers, copy machine, and telephones. Use of computer screen for periods in excess of 30 minutes at a time. Possible eyestrain from extended periods of viewing computer. May be exposed to bloodborne pathogens, infectious diseases, and parasites. Travel throughout the Kalamazoo area is required.
