Henry Mayo Newhall Hospital is looking for a Supervisor of Case Management to join their team.

Henry Mayo Newhall Hospital is a 357-bed acute care, non-profit, community hospital and trauma center located in Valencia, Santa Clarita Valley, California. Henry Mayo Newhall Hospital is known to receive prestigious accreditations and recognition in a multitude of specialty designations for excellence in medicine and healthcare, the hospital offers a full complement of services, including newly opened fitness and health institution, maternity department, cancer program, advanced primary stroke center and spine and joint program. The hospital is an Advanced Primary Stroke Center and a STEMI Receiving Center, as well as part of the Los Angeles County Disaster Resource Center (DRC) network.

The Supervisor of Case Management will work collaborative with the department manager who will:

• Be accountable for the 24-hour management of case management, the management of human, fiscal and other resources.
• The development of personnel, compliance with regulatory and professional standards.
• Unit strategic planning and the fostering of interdisciplinary collaborative relationships within the organization as a whole.
• Responsible for case management of the patients in the main hospital and Emergency Room including: utilization review; providing resources to the patient, family and nursing units; referral to clinical social worker for psycho-social issues and complicated discharge planning issues; under the general direction of the physician;
• Interacting with insurance companies, medical groups, and community resources to ensure timely, efficient use of all services for all age groups.

General Position Requirements:

• Current California RN License
• Current BLS Provider Care
• National Certification (American Case Management Association or Case Management Association of America) Preferred

Education:

• Bachelor’s degree in Nursing (BSN) required
• Master’s degree in Nursing (MSN) preferred
• Case Management national certification preferred

Experience:

• Two years (2) progressive supervisory experience
• Five years (5) progressive nursing experience, with demonstration of competent clinical judgment and leadership ability.

Knowledge and Skills:

• Strong verbal and written communication skills.
• Effective two way communication skills (listening and speaking); strong interpersonal skills with staff, physicians, patients and their significant others.
• Strong leadership and critical reasoning skills in clinical decision-making, planning and organizing, initiative and ingenuity, creativity, independent decision-making, problem solving, and ability to interpret data.
• Ability to work effectively with physicians and multidisciplinary teams toward achieving a common goal in accordance with established timelines.

Pay: $49.90 - $79.84 per hour
