JOB SUMMARY

The Clinical Pharmacist delivers care in a primary care ambulatory care setting to:

• Improve the health and quality of life of patients by optimizing medication therapy
• Improve medication adherence and therapeutic outcomes for patients with chronic health conditions through comprehensive medication management
• Serve as an onsite medication expert for healthcare team members
• Positively contribute to quality measures

Responsibilities

• Optimizes drug therapy directly under a Collaborative Practice Agreement (CPA) and through consultation with other healthcare providers, patients and caregivers.
• Orders appropriate monitoring laboratory studies to gauge effectiveness of disease management or to monitor for potential side effects.
• Provides appropriate medication information and instructs on device usage to patients, patient's families and other healthcare providers.
• Educates patients and healthcare professionals on the safe use of medications.
• Reviews patient records to determine the appropriateness of medication therapy, evaluates patient's health conditions and ensures appropriate treatment.
• Develops effective medication plans to minimize the risk of adverse side-effects and advises on the correct administration of those medications.
• Keeps accurate documentation of medication plans and patient progress.
• Works closely with complex patients to reduce medication confusion thereby potentially reducing admission/readmission.
• Serves on the Value Based Care committee and carries out tasks related to medication adherence and other committee initiatives
• Precepts students and/or residents
  

Complexity of Work

Within scope of job, requires critical thinking skills, decisive judgment, excellent communication, and the ability to work with minimal supervision. Must be able to work in a fast-paced environment.

Minimum Qualifications

Required Qualifications:

• Graduate of an accredited School of Pharmacy Program with doctoral degree of PharmD
• Completion of a PGY1 Pharmacy Practice Residency with at least 50% time spent in ambulatory care, completion of a PGY2 residency in ambulatory care or a related specialty, or completion of three or more years of pharmacy practice experience in an ambulatory care setting.
• Current unrestricted license as a Pharmacist in the State of Wisconsin

Preferred Qualifications:

• Be eligible for participation in all federally funded health care programs, including, without limitation, the Medicare and Medicaid programs, and be eligible to bill for these services incident to a Physician/APP.
• Apply for, obtain and maintain active staff privileges at Progressive as may be necessary for the Pharmacist's practice. The Pharmacist must also comply with all applicable Progressive rules, regulations and directives of the medical staff.
