POSITION SUMMARY: The Clinical Manager supervises, schedules, coordinates, evaluates, and develops clinical staff caring for patients and families in the setting of Inpatient hospice care under GIP, Routine and Respite levels. Responsible for day-to-day operations of the IPU with an emphasis on best practice standards of care to ensure quality care delivery and appropriate patient and family care management within SSRCH scope of services and policies, state, federal, and local laws, and Nurse Practice Act.
PROFESSIONAL QUALIFICATIONS
- Certificate/Degree/License Requirements: RN License to practice nursing in Wisconsin upon hire, BSN and CHPN preferred; valid Wisconsin driver's license upon hire; recertification as required. Must carry car insurance on vehicle used to perform job and have an acceptable driving record. Certified in CPR BLS according to the American Heart Association and/or the Red Cross.
- Experience Requirements: Requires five years of experience as an RN with 3 years' experience working in hospice nursing. Previous experience in management of staff is desirable. Solid understanding of the hospice regulatory requirements is essential.
- Education Requirement: Prefer 4-year bachelor's degree in nursing or 3-year diploma. Will consider 2-year associate degree with appropriate clinical leadership experience.
ESSENTIAL DUTIES
- Knowledge and application of policies and protocols of SSRCH.
- Responsible for the staffing needs of the IPU with consideration of vacancies, PTO, sick Calls.
- Ensures accuracy, completeness, and timeliness of clinical documentation in accordance with SSRCH policies and procedures, regulatory requirements, and industry standards.
- Regularly monitors team performance and coaches on areas to improve efficiency and quality.
- Meets with IPU clinical staff on a regular basis to provide guidance and direction related to patient care to assure that patient and family needs are met, and services are provided according to the plan of care. Assists clinical staff in establishing priorities, setting goals, and evaluating progress toward goals.
- Facilitates regular team meetings to assure coordination of care and regulatory requirements are met. With record keeping of staff meeting agenda and minutes.
- Educates clinical staff on hospice services, policies, and procedures as needed.
- Collaborate with Hospice Outreach Specialists on a regular basis to receive referral information, determine services required, and organization's ability to meet needs.
- Daily assesses LOC changes and initiates process, attends IPU rounds.
- Stays current on available community resources, health care costs, and industry trends through self-education and access to outside education opportunities.
- Collaborates with SSRCH education and quality team regarding appropriate documentation practices, safety, infection control, and overall hospice educational needs.
- Provides leadership to team and support staff in identifying patient needs and opportunities for quality improvement.
- Assists quality improvement teams with data collection for the Quality Improvement Plan established by SSRCH.
- Participates in routine chart audits to help identify educational and quality issues.
- Member of Quality Improvement Committee
- Identifies and implements changes in clinical and/or operational practice based on the findings of the Quality Improvement Program.
- Interprets and enforces human resource policies and procedures in a fair and consistent manner and documents coaching and disciplinary actions in a timely manner.
- Support human resources activities, when necessary, which include interviews and selection of new employees, staff development, performance evaluations, disciplinary actions, and compensation changes
- Promotes personal safety and a safe environment for patients and staff. Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures and regulatory requirements. Assesses safety and environment and is responsible for promotion of safety. Recognizes and responds appropriately to potentially unsafe situations.
- Collaborates with building maintenance to assure regulatory compliance with building codes and standards
- Responsible to ensure all employees have completed mandatory training and continuing education.
- Attends and participates in Interdisciplinary Team meetings.
- Availability during regular business hours and as needed to provide support and assure quality care delivery to IPU patients.
- Participates in Manager on-call rotation
