Clinical Intake Team Lead, Registered Nurse Responsibilities:

• Trains Intake RNs on process and delegates workflow priorities.

• Assists Intake Coordinator Team Lead on new workflow procedures and supporting workflow delegation to Intake Coordination.

• Quick review of referral before approval.

• Monitors complex cases.

• Assists with MD confirmation on complex cases

• Follow up on any Pending Patient delay in SOC request

• Calls hospital/SNF/Rehab on all hold status patients

• Calls for ROC orders and writes order for care to resume

• Completes Tif’s when needed to be done in house

• Verify lab diagnosis.

• Interact with patient and patient’s family to coordinate the start-of-care visit to verify supplies in home etc.

Clinical Intake Team Lead, Registered Nurse Requirements:

• Must have RN Licensure in good standing.

• Minimum of one (1) year of experience in Home Care

• Minimum of one (1) year of experience in a customer service capacity.
