New Avenues to Independence, a non profit organization that provides services to individuals with Intellectual and Development Disabilities, has a GREAT OPPORTUNITY to join our NURSING TEAM!

This position provides clinical coordination of health services for an assigned caseload to ensure the best possible standard of care. Demonstrates and practices the New Avenues to Independence values. Responsible to promptly report to supervisor any circumstances that negatively affect the health, safety, or well being of residents, staff, the agency or its property and equipment.

Qualifications:

• Licensed Practical Nurse with current state of Ohio licensure;
• One year of experience as a LPN in acute and/or long term care setting; prefer minimum one year experience in residential programming of developmentally disabled; OR any equivalent combination of experience and/or education from which comparable knowledge, skills and abilities have been achieved.
• 18yrs of age or older
• Must have an OH drivers license
• High school diploma
