Job Summary and Qualifications

This role serves as the clinical coordinator for resident management and ensures completion of, directs and submits the Minimum Data Set (MDS). Responsible for enhancing the quality of care coordination, patient navigation throughout the continuum of care, and to assure cost-effectiveness of care through case management, education, disease management, utilization review, and support of transitions of care. Clinical case manager includes assessing, planning, and evaluation of residents' individual needs prior to and after admission to skilled nursing. The Clinical Coordinator is responsible for ensuring the timely completion of the MDS and care plan. Is instrumental in orientation and continuing education regarding the MDS and care plan for all care plan team members. Coordinates with residents, family members, direct care staff, specialized therapies/services, and providers as indicated, to facilitate individualized care for each resident. Is knowledgeable in the procedures for initiation of level of care changes. Is able to demonstrate the knowledge and skills necessary to provide care appropriate to the resident served on his/her assigned caseload. The individual demonstrates knowledge of the older adult and possesses the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each resident's requirements relative to his/her specific needs.

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 15 days of employment start date
• Registered Nurse must be obtained within 2 years of employment start date
• Bachelors Degree
