We are a Central Pennsylvania based Non-profit Hospice & Post-Acute Healthcare Organization specializing in home-centered care and we are seeking a talented Full Time Chaplain who will be responsible for providing spiritual support and counseling for HCP patients and their families in regard to end-of-life issues. The Chaplain is a member of the HCP Interdisciplinary Team. The Chaplain is responsible for coordinating spiritual care with community clergy, as appropriate, for HCP patients and families.

This position will provide services predominately within the Schuylkill and Upper Dauphin region of our 7 county wide footprint that encompasses an approximate 60 mile radius from our main office located in Harrisburg PA. This position, though office based in Harrisburg PA, is also considered field based; serving our valued clients wherever they may call home; inclusive of Long Term Care and Assisted Living/Personal Care Communities, Hospitals and in the patient's own residence.

Qualifications:

  • A Bachelor’s Degree in Divinity or related degree and/or all required credentials and ordination for a particular religious denomination. Clinical Pastoral Education (1 unit) is preferred but experience as a hospice chaplain may be accepted in place of CPE experience.
  • Possesses an understanding of various religious faiths and their beliefs regarding death and dying.
  • Experience providing spiritual support to individuals in a health-care setting.
  • Good communication skills and ability to functions as part of a team.
  • Support of the hospice philosophy and mode of care. Willingness to complete the HCP Volunteer Training Program.

Functions:

  • Will participate as member of the Interdisciplinary Team to develop a Spiritual Plan of Care for each HCP patient and their family.
  • Will make home visits or telephone contacts, as requested by patient/family or recommended by the Team, to address spiritual issues with a HCP patient and/or family member.
  • Will coordinate spiritual care with patient’s community clergy; i.e. priest, minister, rabbi, when requested and appropriate.
  • Will make other community referrals, as appropriate, in order to best meet the patient and/or family’s spiritual needs.
  • Will complete all required documentation for patient visits, telephone contacts and referrals. All documentation will become part of the patient’s medical record.
  • Will educate the team regarding spiritual issues and their impact on the patient and family.
  • Will provide in-service education and/or other types of spiritual programs, as appropriate, for HCP Team or community groups.
  • May preside over funerals and/or Memorial services for HCP patients where appropriate and requested by the patient or family.
  • Will participate in HCP’s annual Memorial Service and other community programs as assigned.
  • May be requested to contact or visit a bereaved family member when spiritual concerns are an issue in their bereavement.
  • May be requested to participate in a bereavement support group or education programs to address specific spiritual topics in bereavement.
  • Perform initial and ongoing psychosocial assessments of patients and families admitted for hospice care.
  • Make referrals to appropriate resources upon request of Team or family.
  • Assist the patient and family with care planning, funeral planning, location of care transfers, financial referrals, anticipatory guidance and safety education.
  • Participate in the development of the overall plan of care for each patient/family in conjunction with the team.
  • Attend interdisciplinary team (IDT) meetings and provide updates relevant to patient/family issues.
  • Document in electronic medical record.
  • Assist the Team in understanding the significant social and emotional factors related to the illness, the family structure, and the family dynamics.
  • Acts as a resource and support to other IDT members concerning pyscho- social issues.
  • Participates in in continuing education and organization training to expand knowledge of end of life and palliative care principles and practices.

Schedule:

  • Weekend Rotation (will include Sundays)
  • Holiday Rotation
  • 8 hour shift Monday through Friday - Typically 8:30am to 5:00pm

Experience:
