All Shifts Available: Day, Evening and Overnight

Every Tuesday and Thursday from 9:00am-1:00pm

General Purpose of the Job: Physically assist, train and supervise individuals with developmental disabilities in self-care, grooming, cleaning, shopping, food preparation, household maintenance, eating, bathing and toileting.

Essential Duties and Responsibilities include the following. Other duties may be assigned:

  • Providing physical support for individuals with daily activities and personal hygiene, including bathing, dressing, getting out of bed, using the toilet, walking, standing, or exercising.
  • Noting observations of individuals behavior, including complaints, or physical symptoms to nurses.
  • Teach individuals the elements of good nutrition and menu planning.
  • Attend to personal self-care/other needs of individuals unable to care for themselves.
  • Train and teach individuals to achieve greater independence in all areas of daily life.
  • Accompany individuals on community outings while teaching community skills.
  • Implement and document goals, plans, or methods as provided.
  • Attend in-service presentations as mandated by management.
  • Will be required to drive agency vehicle/van on occasion in order to escort individuals to various appointments/events.
  • Comply with all agency policies and protocols as dictated by management.
  • May be required to report to work at different UCP Suffolk County sites or provide individuals with companionship during periods of hospitalizations.
  • Advocate on behalf of individuals and their families.
  • Ensure and protect the rights of all individuals, keeping all individuals free from abuse.
  • Complete incident reports and all paperwork timely and fully.
  • Accompany individuals to medical appointments and communicate accurately between medical personnel and management.
  • Assumes other activities and responsibilities from time to time as directed.
  • Stay up to date on CNA training.

Education and/or Experience

State Certified Nursing Assistant certification. High school diploma or general education degree (GED) preferred. Experience with the developmentally delayed and disabled preferred.

Certificates, Licenses, Registrations

Valid clean NYS Driver's License required throughout employment. Completion of Agency Orientation and Residential Training programs.
