Job description
This full-time position will serve as the first point of contact for clients presenting for primary health care services at Heritage who have a diagnosis of serious mental illness and other chronic health condition(s). They will provide assistance to nurses with care coordination, rooming patients in the clinic, and clinical monitoring of persons served.
Responsibilities Include: (but are not limited to):
- Check-in of patients, obtaining necessary patient information to file and update patient records, and ensure all forms and consents are completed
- Prepare patients for examination, take vitals, and record patients' health history
- Check-out of patients, assist with referral processing, and arrange laboratory services
- Must have a firm grasp on medical practices, administrative processes, and organizational policies
- Ensure confidentiality of clients is maintained at all times
Education and Experience:
The ideal candidate will have excellent interpersonal skills, and the ability to communicate clearly and effectively in a courteous and friendly manner are required. Candidates must be detail oriented, and possess excellent organizational skills. A minimum High school education or equivalent, as well as completion of an accredited medical assistance certification program. (A Certified Nursing Assistant may be considered for this position.)
Valid driver’s license, reliable means of transportation, and proof of current automobile insurance are required.
