Position Summary

Provides culturally sensitive support, education, and case management services to address the specialized needs of pregnant and newly parenting women with substance use disorders (SUD), their children and their families. Responsibilities include providing assessments, referrals, follow-up, and other required services. Services will be provided in Montgomery, Buck’s and/or Delaware Counties.

Essential Functions

  • Provides a brief case management intervention to identify needs of women with SUD and help set goals for a healthy prenatal and postpartum experience.
  • Provides assessments, referrals and follow-up support through responsive home, community and/or virtual visits and regular phone/text contact.
  • Supports clients in goal setting for their health, social service and recovery needs.
  • Refers participants to long-term home visiting programs and other appropriate community services for on-going support.
  • Supports clients in addressing substance use issues. Provides information on treatment, harm-reduction, and MAT options.
  • Client accompaniment to appointments as needed to ensure successful engagement.
  • Attends orientation and ongoing training sessions on maternal and child health and welfare, substance use, parenting, and PA mandatory reporting.

Knowledge, Skills, and Abilities

  • Significant knowledge of and sensitivity to issues facing the community served.
  • Knowledge and skill pertaining to the delivery of substance abuse services.
  • Experience in client management procedures for determining eligibility, assessing needs, identifying resources, making referrals, following up, and documentation.
  • Compassionate, courteous, collaborative, and culturally competent interpersonal skills.
  • Effective and accurate documentation of case notes and client data.

Experience, Education, and Licensure

Required

  • Associate degree in Social Work, Public Health or related field
  • Experience working with substance abuse
  • Valid driver's license

Preferred


  • Bachelor’s degree in Social Work, Public Health or related field
  • 2+ years experience providing direct services in a community outreach setting
  • Experience working with maternal and child health issues
