JOB PURPOSE:
Ensure delivery of services to aide clients in accessing behavioral health services by supporting the prevention of homelessness, offering housing assistance search and resource locating to support financial assistance needs to decrease crisis situations, the traumatic impact of homelessness and increase access to care. Case Manager will focus on the prevision of services, success in utilizing the service and follow up to confirm services resulted in the increased quality of life and is supporting the client with the highest possible ability to function within the community.
