IN HOME MEDICAL CASE MANAGER: A CAREER THAT SERVES HUMANITY!

We are offering a $5,000 signing bonus, contingent on passing probation!

Giving Home Health Care is focused on providing In-Home Health Services for those who have worked in the U.S. nuclear industry.

We have an immediate need for a full-time, office based, Case Manager

• Work on new patient admission packages;
• Help and prepare plans of care;
• Daily Patient Case Management documentation and record keeping;
• Be with patients at Doctor appointments - advocate and explain program requirements for the patient and medical providers;
• Weekly conversations with all team members, Nurses, CNAs and HHAs on each case you manage; and
• Perform periodic home site visits.

We seek persons who communicate well and possess a warm personality. The incumbent must have well developed reading, writing, and computer skills; able to manage multiple projects.

We prefer minimum Associate Degree or equivalent experience. One year experience in medical case management or health care/medical is appreciated. Knowledge of medical terminology or medical experience already known or learned is needed to be an effective Case Manager.

Pre-employment assessment testing is given to qualified applicants.
