This is a REMOTE position. BHS is seeking candidates throughout the US. Applicants outside of Maryland will be considered for this role.
Position Summary:
The Case Management Referral Coordinator administratively assists in researching and connecting with referral resources on behalf of participants accessing services. In addition, they will document outreach and facilitate coordination with behavioral health treatment providers, facilities, and other sponsored benefits as appropriate.
Position Duties & Responsibilities:
In a manner consistent with supporting BHS’s Mission Statement the Case Management Referral Coordinator shall do the following:
  • Accepts referrals for service requests and connects with appropriate resources
  • Responsible for the tracking, coordination, and communication of referrals
  • Ensures complete and accurate documentation of referral requests, including client demographics and current insurance information
  • Reviews details and expectation of referral with Care Coordinators
  • Ensures that referrals are addressed in a timely manner
  • Works collaboratively with team to ensure the highest level of customer service
  • Responsible for administrative management of their caseload as well as their identified Care Coordinators, inclusive of:
    • All email follow ups with participant and providers for standard touchpoints as determined by promises
    • Checking and entering all voicemails/emails into dynamics and setting appropriate follow ups
    • Research community resources/treatment providers
    • Sends any necessary materials to provider
    • Closing case and ensuring all fields appropriately completed
    • Responsible for ensuring that all customer promises related to case management are followed
  • Manage task list to ensure resources are sent to participants within the parameters of BHS’ promises
  • Maintains accurate and complete client information in our customized computer program
  • Connects clients to appropriate employer and/or community benefits
  • Answers a multi-line telephone system and documents calls
  • Accurate and timely processing of all Contact Center voicemail and general email accounts, task list and BHS faxes
  • Assists with participant needs as requested by Care Coordinator
  • Enters case data into Dynamics
  • Monitor various incoming communications and route appropriately.
Other duties as assigned
Education, Training and Experience:
  • High School Diploma required; Associates Degree is preferred
  • One to three (1-3) years’ experience in case management; using a case management platform/HER.
  • One to three (1-3) years’ experience working in a call center environment.
  • Proficient in Microsoft Office applications including Word, Excel, and Outlook
