Details about this exciting opportunity:

• Full-Time Care Manager to work in our Mary J. Blige Center in Yonkers, NY.
• Compensation: $40,000 - $47,000/year.

Responsibilities include (but are not limited to):

• Conducting initial and ongoing assessments.
• Collaborating with clients and staff to develop a plan of action to assist with implementing change and meeting the clients most urgent needs, such as housing and food insecurity, obtaining affordable child care, etc.
• Coordinating with team members and following up on referrals.
• Link with outside services, primary care management.
• Participating in multidisciplinary care planning meetings.

If you have the following:

Qualifications:

• An Associate’s degree, or BA/BS in related field, with 2 years’ experience.
• Ability to travel to various community sites for outreach.
• Bilingual (English-Spanish) highly preferred.

• Knowledge of community resources.
